The Dutch dental association NTVT is hosting a webinar on deep adhesive restorations, exploring when teeth with deep cavities can still be restored adhesively and when extraction with immediate implant placement becomes the better choice. The session, featuring dr. Rijkje Bresser and dr. Kees Heijdenrijk, addresses the clinical and biological complexities of deep restorations.

Adhesive restoration of deep cavities

As cavities deepen, maintaining isolation for adhesive restorations becomes increasingly difficult. Dr. Bresser covers practical strategies for deep direct and indirect restorations, including how to manage subgingival cavities where rubber dam inversion is no longer possible, techniques for keeping the field dry during indirect work, and the use of Teflon and additional dam clamps in complex cases. She also discusses biological width, tooth and composite pretreatment, bonding principles, and matrix systems. Deep Margin Elevation (DME) can extend the range of adhesive treatment, but the seminar examines when this technique reaches its limits and the biological consequences of working at or beyond those limits.

Immediate implants when extraction is indicated

Dr. Heijdenrijk presents cases where DME is no longer possible and extraction becomes necessary. For patients wanting a complete dental arch, immediate implant placement for a crown offers a viable alternative. He demonstrates immediate implant placement in the molar region, outlines the advantages of the technique, and clarifies the clinical criteria for when immediate implantation is and is not suitable.
